Affidavits of destroyed seized beverages.

Any confiscated alcoholic beverages not in saleable condition are to be destroyed by the ABC Board after final diposition of case is obtained from the court. This series consists of Affidavit Forms completed by the District Supervisor to document the destruction of the confiscated merchandise. Information available on the affidavit includes county name, supervisor's name, date and location the merchandise is seized, date and location where the merchandise is destroyed, and signature of the supervisor. The Affidavit is then notarized and filed with the division's central office.
